Consumer Workshop: continuity of care / shared care guideline
Facilitated by the Department of Health’s Mental Health Unit (MHU) and hosted by CoMHWA.
Within the past five years:
- have you been discharged from a public mental health service?
Public mental health services are those that are funded and managed by the government, such as a mental health ward at a public hospital or a community mental health service. - has your care been transferred from one public mental health service to another?
- have you moved from child to adult mental health services or from adult to older adult mental health services?
- have you experienced a shared care arrangement?
Shared care arrangements are arrangements where aspects of care are shared between different providers at the same time; for example, where a consumer attends a community mental health service for therapy, sees a private psychiatrist for medication, and has a support worker funded through the National Disability Insurance Scheme.
If you answered yes to any of the above questions, then we invite you to attend a face-to-face workshop on Wednesday 16th October 2024 from 10am to 1pm at CoMHWA, 12/275 Belmont Avenue, Cloverdale, WA 6105.
Morning tea will be provided, and you will be paid for your time.
Purpose of the workshop:
The MHU is developing a guideline to support continuity of care in relation to transfer, discharge, and shared care arrangements.
Workshop participants will be asked to:
- review the draft guidelines (that will be provided 2 weeks prior to the workshop) and identify any gaps and/or areas of concern
- discuss experiences, both positive and negative, of transfer, discharge and shared care arrangements
- discuss what makes transfer, discharge and shared care ‘good’ from a consumer perspective
- agree on preferred language for the guidelines and surveys
- assist with refining a survey which will be used to gain a broader view of consumer experiences in WA.
